Health Insurance Information & Consent
The Mark West Union School District is requesting that families voluntarily provide health insurance information for school-related activities such as field trips, overnight trips, and athletics, and to support reimbursement for eligible school-based behavioral health services, including services provided through the Children and Youth Behavioral Health Initiative (CYBHI) Fee Schedule Program.
Providing insurance information and consent allows the District to seek reimbursement from insurance companies when eligible services are provided to students. Families will not be charged copayments, deductibles, or experience increased insurance premiums as a result.
In order to do so, it will be helpful for you to have your student’s health insurance information readily available when completing the Parent Data Confirmation process as you will be asked to provide the following information:
- Insurance Type
- Insurance Carrier
- Effective Date (use today’s date)
- Policy Number or Member ID
- Group Number (if known)
- Subscriber First Name
- Subscriber Last Name
If you choose not to provide health insurance information, you will be given the option to select “Decline to Provide” in the Insurance Type field and enter the date in the “Effective Date” field.
